Organizational coding for the Report of Claims

Policyholders desiring to pinpoint injuries by department or work locations may do so by establishing organizational codes.
Establish a simple coding system
Policyholders can identify any particular characteristic that they wish to isolate for statistical purposes. The codes must
be 12 characters or less and they can be alpha or numeric. For example, a policyholder could use 01 for Shop and 02 for
Warehouse, or the policyholder could simply spell out the names such as Shop for shop and Warehouse for
warehouse. The policyholder should select codes that are easy to use and remember in order to ensure consistency when
entering those codes in the Employer section of the First Report of Injury or Illness (FROI) form (see number 3 on the sample).

Review the codes on the Report of Claims
Entering the correct code for every claim submitted will result in the Report of Claims being compiled with claims separated
into the organizational codes selected. This will assist in identifying work locations, departments, or supervisors having
safety problems and assist in determining potential corrective actions.
