How to apply for a policy

Four ways to apply

There are four ways to apply for a policy with the State Insurance Fund:

  1. Download, complete, and upload an application (see below).
  2. Download, complete, and e-mail as an attachment to PolicyApplications@IdahoSIF.org.
  3. Call us at 208-332-2100 or 800-334-2370 and ask for the application line.
  4. Contact an agent of your choice.

The cost of your policy is the same whether you contact the State Insurance Fund directly or choose to be represented by an agent.

Download an application

Using Word

If you use Word, you can download our policy application form, complete it on your computer, and return it to us by uploading it.

To download:

  • Click on the link below.
  • Go to the menu bar of your browser, and click on "File" followed by "Save As".
  • Save the file in the folder of your choice on your computer.

Upload an application 

You can upload a completed policy application directly to the State Insurance Fund. This is a secure sockets layer transmission with an encrypted connection. To upload the application:

  1. Download the application form from our Web site, complete it, and save it.
  2. Click on Upload completed application to the State Insurance Fund Web site.
  3. Click Browse to locate the completed application form on your computer.
  4. Click "Secure Submit" to upload the completed form.
  5. Be sure to save the application for your records.

Using an Adobe Acrobat PDF

Don't use Word? You still can download a policy application in PDF format, print it out, complete it neatly, and mail or fax it to us. Download Adobe Acrobat Reader, which reads PDFs, is available for free.
