Initial Notification of Claim
On time-loss claims, a letter is sent to the claimant advising them a claim has been received, who they should
contact if they have questions, and providing general information regarding claim processing.
Compensation Payment Report
When a claimant is receiving compensation payments, a compensation payment report is available in the secure area of the Web site. The report shows the date and the amount of each
compensation payment.
Notice of Change of Status letter
A letter is sent to the injured worker, with a copy to the employer, whenever there is a change in the status of the compensation
payment.
Permanent Impairment Award
A letter sent to the injured worker, with a copy to the employer, advising of a permanent partial impairment award as rated by
a physician. It is a recount of benefits paid to date as well as future payments for the permanent partial impairment.
Report of New Claims
At the request of the policyholder, the State Insurance Fund will send a monthly report showing the claims registered during the prior month.
