Policyholders who are registered with the State Insurance Fund can access policy
information in the secure area of our web site. The type of policy information
available in the secure area is detailed below.
Current policy period details
This area includes pertinent information related to your current policy period.

the name, telephone number, and e-mail address of the agency assigned to your
policy (if you use an agent)

policy name and number
-
policy anniversary date
-
estimated annual premium
-
outstanding payroll reports (if any)
-
outstanding amounts (if any)
-
policy status (active, pending cancel, etc.)

current balance
-
policy address and telephone numbers
-
policy effective and expiration dates
-
deposit amount and billing plan
-
e-mod and e-mod history (if applicable)

endorsements (if any)
-
limits of liability (applicable only to Part Two of the policy)

Renewal details
Sixty days prior to the renewal date, information for the renewal policy will be
displayed in this area. This includes all of the above information plus:

class codes and rates effective on the renewal date
-
estimated payroll by class code and location
-
estimated premium by class code and location
-
total estimated annual premium
Current rating details
This area includes the rating information for your current policy period.

Financial transactions
This area contains a list of the policy financial transactions for the current
policy period plus the last six months of the expired policy period. Financial
transactions include:

Payroll Request--this includes the payroll start and end date and the date the
payroll report was sent to the policyholder.

Payroll Received--this includes the payroll start and end date and the amount
of premium the payroll report generated. To see the payroll figures reported
and the subsequent premium due by class code, click on "View" in the far right
column.

Payment Received--this includes any debit or credit applied to your account. To
see the details of the debit or credit as they appeared on your statement,
click on "View" in the far right column.

Payroll Revision--this includes any change in the amount of premium due because
of revisions to the payroll amounts previously reported.
Report of claims
This area displays the claim information for the policy by policy year.
Information for each claim includes:

date of injury
-
claim number
-
claimant name
-
claim disposition (accepted, pending, denied)
-
claim status (open or closed)
-
nature of the injury
-
body part injured
-
total medical payments made
-
total compensation payments made
-
list of each compensation payment and the date paid
-
outstanding reserve amount
-
incurred losses (payments + reserves)
-
examiner name, telephone number, and e-mail address.
Claim information can be exported to a spreadsheet program (like Excel) and then
sorted and analyzed as desired.
Loss Experience Analysis Report (LEAR)
This area displays premium and loss information for the current policy year and
up to five past policy years. Information for each policy year includes:

dividend received (if applicable)

To see a list of individual claims for the policy year, click on "View" in the
Claims Count column.
